d. Affidavits

Required Documents:

To participate in the Action, you must submit the following supporting documents:

Copy (Both Sides) of Identity Card
Printing of Individual/Legal Person Registry Details from the IAPR
Curriculum vitae (maximum 2 pages)
Copies of Declared Academic Qualifications
Required SME Declaration
(attached by existing businesses / entrepreneurship & innovation support bodies)
Responsible Declaration of Acceptance of the Terms of the Action
(attached by all interested parties)
Declaration of De Minimis Aid Received
(declaring the amount of aid received under the above regulation over the last three years - to be attached by existing businesses / entrepreneurship & innovation support bodies)
